- Conduct psychological assessments and formulate behavioral health interventions within a primary care setting; assist with implementation and monitoring of treatment plans with Primary Care Managers and Behavioral Health Case Managers.
- Provide timely and concise feedback to PCMs regarding consultation findings and recommendations; provide concise documentation of care and recommendations within the patient’s medical record.
- Provide focused follow up care, including relapse-prevention education; triage and refer patients to Specialty Behavioral Health Care when appropriate.
- Evaluate treatment effectiveness and make appropriate adjustments; provide on-going consultation services for a sub-set of patients who require on-going monitoring.
- Develop, teach, and provide oversight for classes that promote education and skill-building to enhance psychological and physical health; promote behavioral change related to improved health status.
