Further details regarding the schedule will be provided during the application process.Position Purpose:The Clinic Embedded HIV Engagement Specialist (CEHES) will work as part of the Data 2 Care Team implementing strategies for patient identification and engagement. This position plays a critical role in relinking and retaining patients in HIV medical care and connecting to supportive services to address barriers to care. The CEHES serves as the primary service delivery staff for this program. This includes outreaching and locating patients who are out of care, assisting patients with transportation services to clinic appointments, arranging translation services as needed, providing system navigation, delivering health education, offering brief counseling, and encouraging participation in other specialized programs (e.g., LGBTQ+, substance misuse, and mental health treatment).This position requires close collaboration with clinic embedded programs including Primary Care teams, Linkage to Care, Medical Care Coordination, and more to relink and retain patients in HIV medical care.Principal Responsibilities: * Implement patient outreach, identification, and engagement strategies.
- Conduct outreach including in-person home visits, shelter visits, and last known locations– including homeless encampment sites. Utilize various data sources (Clarity, Case-Watch, Next-Gen, Inmate locater, and more to identify patient’s last known location.
- Establish and maintain regular communication with patients to build rapport and support their needs.
- Coordinate referrals and linkages to HIV medical care, MCC, and other support services.
- Participate as a member of the health care team to gather information on patients’ home environments, family relationships, and other factors impacting illness, treatment, and recovery.
- Use electronic medical record queries to track patient appointments.
- Document referral linkages and patient monitoring activities in Electronic Health Record and DHSP databases.
- Assist patients in addressing socioeconomic barriers to HIV care, such as housing, transportation, financial support, and benefits coordination.
- Maintain accurate patient files to support continuous quality management reviews by DHSP.
- Provide systems navigation, health education, and brief counseling to support patients in linking or re-engaging in care and achieving viral suppression.
- Collaborate with data analysts to ensure accurate record-keeping and reporting, maintaining secure data collection and exchange processes.
- Promote and encourage participation in services and programs for specialized populations (e.g., LGBTQ+, substance misuse).
- Participate in monthly DHSP-hosted training and technical assistance activities related to program responsibilities.
- Monitor and support clients to sustain engagement and retention in HIV care.
- Perform other duties as assigned.
Requirements: * Bachelor’s degree in Social Work, Psychology, or an equivalent social science degree; and at least one year of experience in caring for LGBTQIA+ PLWH.
- Reliable transportation and possess a valid California Driver’s License.
- Minimum of two years of experience working in community health or social services.
- Evidence of recent education in LGBTQIA+ PLWH.
- Strong understanding of healthcare disparities and social determinants of health.
- Experience in providing culturally competent care to diverse populations, including LGBTQIA+ individuals.
- Excellent computer literacy skills, including proficiency in word processing, data inquiry in County databases, and data entry.
- Excellent communication skills, both verbal and written.
- Ability to maintain confidentiality and adhere to ethical standards in patient care
Preferred: * Master’s degree in Social Work, Psychology, or a related social science field.
- Bilingual proficiency in English and Spanish.
- Six (6) months of experience in a clinical setting.
- Proficiency in researching, identifying, and accessing community referrals.
- Knowledge of substance use, homelessness, mental illness, and other issues impacting the client population.
