The Clinical Intervention Specialist is primarily responsible for the provision of intensive care coordination, treatment planning, wraparound/coordination of intensive support services for a multi-disciplinary, hospital-based violence intervention program, dedicated to reducing re-injury and mortality rates among youth and adults by providing intentionally injured patients with support during their hospital stay and beyond. This position will provide on-site bedside visits and intensive in-home and community follow up services. Clinical Intervention Specialists will provide case management, mentoring, advocacy, and provide therapeutic clinical interventions as needed. Clinical Intervention Specialists will maintain intensive follow up contact with patients and their families, as well as other service providers through frequent home visits and telephone contact. They will offer wraparound services to patients and their families based on the unique individual of the needs of the patients enrolled in the program. They will be responsible for developing treatment plans, coordinating care and documenting contacts and service coordination consistently, accurately, and timely in the records of the patient.What you will do
- Enrolls new patients and facilitates assessments to help determine the need for therapeutic medical, psychosocial, psychiatric, financial, spiritual, and other community based services.
- Serves as intervention specialist who will design, execute, and assess individualized interventions for the patients enrolled in the program. Works with the HVIP team to identify the target population; those patients who are admitted due to violent injury and seeks to engage these patients in services for ongoing care. Must be knowledgeable and resourceful to other service providers in the community to assist with care coordination and wrap-around treatment services. Recognizes patients are part of a larger system, to include school, family, work, and community which impacts issues of treatment and recovery. Provides continued collaboration between agencies for ongoing care coordination.
- Works closely with hospital and community providers to connecting patients to appropriate services. Provides linking to any necessary programs, facilitates the coordination of services being provided, helps to arrange necessary appointments, provides personal support, and crisis intervention when needed to patients enrolled in the program. Assist those who may need help navigating support services, completing applications, or following up with services to remain engaged and connected to support services.
- Understands and works with those who are at-risk for violence and their families. Connects with those enrolled to engage and maintain services. Works collaboratively with families and community agencies, recognizes and pro-actively seeks consultation with the HVIP team, managers, and stakeholders as needed. Assesses unique situations, identifies causes, gathers relevant information, generates possible solutions, and helps make recommendations to address or resolve the problem(s). Maintains care and coordination for a caseload of patients enrolled in the program.
- Participates in interdisciplinary treatment team meetings, sharing information with team members. Attends trainings, in-services, and keeps up to date with evidence based practices for serving the target population.
- Provides direct care to patients by assessing their treatment needs; developing, monitoring, and evaluating treatment plans and patients progress utilizing a variety of interdisciplinary approaches as well as care coordination with an interdisciplinary team. Provides community based services to those who have experienced intentional violent injury through ongoing assessments, crisis intervention and supportive counseling services. Directs client contact in hospital setting as well as field work to include community based outreach, interventions, coordination, linking, support for those enrolled.
QualificationsEducation
- Bachelors Degree, (Preferred)
Experience
- 3-4 years Healthcare and/or community programs experience (Required)
- Knowledge of hospital systems/healthcare and community resources (Preferred)
Licenses and Certifications
- Valid Drivers License Required
