Job description: DescriptionThe Community Health Corps (CHC) Case Manager (Administrative Special Services II) ARPA primarily serves to stabilize and facilitate resource navigation services for Detroit residents. Where needed, the Community Health Corps (CHC) Case Manager will provide direct connections in the form of application assistance and warm handoffs to residents with significant household barriers. Beneficial documentation assistance, including ID, social security cards, birth certificates, proofs of income and/or proof of benefits for all household members is provided by the Community Health Corps case manager. If income is a barrier, the Case Manager will utilize internal partnerships to gain employment or SSI/SSD benefits within a defined case management period. Upon completion of the case plan, the resident will be connected to local resource organizations for long term support.The Community Health Corps (CHC) Case Manager will maintain a caseload of 75 or more households, with the expectation to complete resource navigation training and connection with the resident within 180 days. Daily contact with households within the caseload is required. Duties include closely maintaining a record of contact and resources provided to households, case notes, ID and documentation support delivery.Examples of Duties

Minimum QualificationsEducation
Bachelor’s degree from an accredited college or university, with major course work in business administration, pubic administration, public policy, economics, finance, accounting, urban studies, information technology, or a case management, health, social services or social work-related field or four years of professional experience.Experience
Two years of professional experience involving the systematic review of financial data or one (2) years of experience in customer service and health, human or social services related field.Equivalent combinations of education and experience may be substituted to meet the education and experience requirements of this position.Supplemental InformationAppointment term is based on availability of grant funds.Medical Plan Offering for Administrative Special Services Employees

This plan provides you with the opportunity to receive an enhanced level of benefits and pay less out of your paycheck for your medical plan. There are two important requirements that you should consider and understand if you should decide to enroll in this plan. You will have to choose a primary care physician that serves within the PCP Focus network. This primary care physician coordinates your care. The network includes over 22,000 Primary Care Physicians.
