We are currently looking for a Clinical Data Coordinator & OASIS Rviewer to join our team. The position would primarily work remotely and attend training at our Neptune location. This position would be responsible for the review of the admission, resumption, recert and discharge assessments for Medicare, Medicaid, Medicare/Medicaid HMO cases to ensure that the information flows logically and corresponds to the Plan of Care and medication profile as well as communicating with the clinician and ensuring appropriate documentation.#INDNURSESResponsibilities

- Collaborates with clinicians to ensure documentation of OASIS assessments, Plan Of Care, and medication profile follows logically and accurately
- Ensures compliance with VNACJ policies and follows them in the process of decision making
- Develops and maintains an effective working relationship with all members of the health care team
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
Qualifications
- RN with Bachelor’s degree in Nursing
- OASIS Certification required
- 3-5 years of home care experience
- Strong knowledge of OASIS and Medicare/Medicaid guidelines
- Strong computer skills
- Excellent verbal and written communication skills
- Excellent interpersonal skills
